Bhubaneswar: In a striking reminder of nature’s rarest anomalies, a 22-year-old first-time mother from Odisha’s Kandhamala district has given birth to male conjoined twins joined side by side with two fully formed heads and necks sharing a single torso, arms, legs and genitalia.
The premature infant, delivered by lower-segment Caesarean section under spinal anaesthesia at MKCG Medical College and Hospital in Brahmapur around 32 weeks of gestation, weighed 2.25 kg and was immediately shifted to the Special Newborn Care Unit for intensive monitoring.
Doctors have diagnosed the condition as dicephalic parapagus, an extremely uncommon form of conjoined twinning that occurs roughly once in every 300,000 births. The newborn has two separate spinal columns yet shares a single heart and liver, one pair of limbs, a single genitourinary tract with male external genitalia, and lacks an anal opening. Respiratory complications have already set in, leaving the prognosis uncertain as a multidisciplinary team evaluates the extent of shared anatomy and organ function.
Hospital superintendent Professor Sudeepa Das described the case as possibly the first of its kind reported at the institution, while paediatric surgery head Dr Manas Ranjan Sahu noted that survival hinges on the viability of vital organs and the management of post-birth complications.
The mother, a resident of Tumudibandh area whose husband works as a daily-wage labourer, had been referred to the tertiary facility for specialised care. Conjoined twins form when a single fertilised egg fails to separate completely, and dicephalic parapagus variants present particularly complex challenges because the twins are fused laterally and often share critical thoracic and abdominal structures. Most such infants are stillborn or die shortly after birth; long-term survival is exceptional and usually depends on each twin possessing sufficiently independent hearts and other vital systems.
This latest birth has inevitably drawn parallels with Odisha’s earlier high-profile case of Jaga and Balia, the craniopagus twins from Milipada village in the same Kandhamal district who were born joined at the head in 2015. After a landmark multi-stage separation surgery lasting more than 40 hours at AIIMS New Delhi in 2017, Balia later succumbed to septicaemia in 2020 while Jaga continues rehabilitation.
