Silent Epidemic: Lead Exposure Steals Futures Across South Asia

Lead exposure remains one of the most pervasive yet preventable environmental health threats facing South Asia. Decades after the phase-out of leaded petrol, the region continues to shoulder a disproportionate share of the global burden. UNICEF and Pure Earth estimates indicate that of the roughly 800 million children worldwide with blood lead levels above the level of concern, nearly half live in South Asia. India alone accounts for an estimated 275 million affected children, Bangladesh around 35 million and Pakistan tens of millions more.

A comprehensive 2026 review of blood lead levels across India, Bangladesh, Pakistan, Nepal and Sri Lanka found that concentrations frequently exceed the US Centers for Disease Control and Prevention reference value of 3.5 µg/dL. Mean levels in Indian children have been reported around 10.4 µg/dL in pooled analyses. In high-risk industrial areas of Pakistan, as many as four in ten toddlers show elevated levels, rising to 88 per cent in some sites such as Hattar. In parts of Dhaka, studies have detected lead in virtually every child tested.

Sources That Persist Long After Petrol Ban

The primary pathways have shifted from vehicle emissions to a complex mix of informal industry, consumer products and contaminated food. Informal recycling of used lead-acid batteries stands out as a dominant source. Workshops often operate in densely populated neighbourhoods without emission controls, releasing lead into soil, dust and air. Battery manufacturing and paint production also contribute significantly; testing in Pakistan has found many solvent-based paints far exceeding legal limits.

Dietary routes compound the problem. Rice, vegetables and especially spices frequently show elevated lead concentrations. Lead chromate has been used to adulterate turmeric and other spices for colour, producing levels hundreds of times above safe thresholds in some samples. Traditional cosmetics such as kohl and sindoor, glazed ceramics, aluminium cookware, toys and certain herbal remedies add further exposure. Contaminated soil and household dust, particularly near industrial sites, complete the multi-pathway picture that characterises the region.

Irreversible Harm to Developing Brains and Bodies

There is no known safe level of lead exposure. In children, even low concentrations impair neurodevelopment. Effects include reduced IQ, shortened attention spans, learning difficulties, behavioural problems and increased risk of aggression later in life. Stunting, anaemia and weakened immunity are common co-morbidities. Lead crosses the placenta, placing unborn babies at risk of adverse birth outcomes.

Adults face elevated risks of hypertension, cardiovascular disease, renal and hepatic dysfunction. Globally, lead exposure is estimated to contribute to more than 3.5 million deaths annually, with cardiovascular impacts forming a major share. South Asia’s high population density, rapid urbanisation and industrial growth amplify these risks for both children and adults.

Heavy Economic Toll Across the Region

The economic consequences are staggering. Modelling linked to World Bank analyses estimated that South Asian children under five lost around 230 million IQ points in 2019, with associated future income losses equivalent to roughly 3.5 per cent of regional GDP. In Bangladesh, one detailed assessment put the combined health burden at US$28.6 billion in 2019, or 6 to 9 per cent of GDP. Pakistan faces similar losses estimated in the range of 6–8 per cent of GDP from cognitive impacts alone. India has reported annual economic losses from childhood lead exposure approaching several percentage points of GDP.

These figures capture only part of the picture. Healthcare costs, special education needs and reduced workforce productivity compound the burden on already strained public systems and families.

Uneven Progress and Emerging National Responses

Some countries have begun systematic measurement. Bhutan completed its first national blood lead level survey in 2024 with UNICEF support, linking elevated levels to anaemia and developmental delays. Bangladesh and Pakistan have conducted targeted studies in high-risk urban areas, while Indian researchers continue to map state-level variation. Yet national screening programmes remain limited, and regulatory enforcement of paint standards, battery recycling and food safety often lags behind the scale of the problem.

International agencies stress that lead poisoning is entirely preventable. WHO has called for accelerated action during the ongoing International Lead Poisoning Prevention Week, noting that fewer than half of countries maintain legally binding controls on lead paint. Practical steps include stricter product standards, formalisation and environmental controls on battery recycling, improved food safety monitoring, public awareness campaigns and expanded blood lead testing capacity.

South Asia’s lead crisis is not inevitable. Experience elsewhere demonstrates that decisive regulation, industrial formalisation and community-level interventions can sharply reduce exposure. The region’s experience underscores a broader truth: environmental health threats that appear local in origin carry global implications through lost human potential, reduced economic growth and heightened inequality.
